If the entity requesting a certificate requires to be named as an additional insured or there is any special contractual wording required, please call our office at 781-237-1515.

Please include the information requested in the PDF (i.e. Insured, Description of Operations/ Locations/ Vehicles, etc., and Certificate Holder) and email it to us. In addition, please consider the following question:
